First Class Transitional Care
for Young Adults
At Standshill, we are passionate about bridging the gap between children’s and adult services, offering trauma-informed, person-centred support to care-experienced young people, including care leavers and those at risk of placement breakdown. Our team brings extensive experience from children’s residential care, alongside a deep understanding of mental health, safeguarding, and transitional care planning.
We understand that many young people face challenges at this stage of life — from emotional regulation to independent living skills — and we believe in creating safe, stable, and empowering environments where they can develop confidence and thrive.





Our Transitional Care Model
Currently, we offer non-regulated supported living placements for young people aged 17+ and care leavers aged 18+. Our model includes:
- Keyworker-led support
- Outcome-focused planning
- Independence and tenancy skill development
- Support with emotional wellbeing and community integration
- Multi-agency working and family liaison
While we are in the process of completing our CQC registration (for personal care), our supported living offer is fully aligned with best practice and the principles of the Single Assessment Framework.
